RESUMO

ABSTRACT Objective: To determine the efficacy of anesthetic transforaminal nerve root block in patients with sciatica secondary to lumbar disc herniation through a prospective observational study. Methods: The study included 176 patients from a private clinic undergoing transforaminal injection performed by a single spinal surgeon. The patients were assessed after two weeks, three months and six months regarding to the improvement of the pain radiating to the lower limbs. In case of persistent symptoms, patients could choose to perform a new nerve root block and maintenance of physical therapy or be submitted to conventional microdiscectomy. Results: By the end of six-month follow-up of the 176 patients, 116 had a favorable outcome (95 after one block and 21 after two blocks), and only 43 required surgery. Conclusion: The results of our study suggest a positive effect of transforaminal block for the treatment of sciatica in patients with lumbar disc herniation.


RESUMO Objetivo: Determinar a eficácia do bloqueio anestésico transforaminal nos pacientes com ciática secundária à hérnia de disco lombar, por meio de um estudo prospectivo observacional. Métodos: Foram incluídos no estudo 176 pacientes de uma clínica privada submetidos à injeção transforaminal executada por um único cirurgião de coluna. Os pacientes foram reavaliados após duas semanas, três meses e seis meses quanto à melhora da dor irradiada para os membros inferiores. Na persistência dos sintomas, os pacientes poderiam optar por realizar um novo bloqueio e manutenção da fisioterapia ou serem submetidos à microdiscectomia convencional. Resultados: No final do seguimento de seis meses dos 176 pacientes, 116 apresentaram evolução satisfatória (95 após um bloqueio e 21 após dois bloqueios) e apenas 43 necessitaram da cirurgia. Conclusão: Os resultados do nosso estudo sugerem um efeito positivo do bloqueio transforaminal para o tratamento da ciatalgia nos pacientes com hérnia de disco lombar.


RESUMEN Objetivo: Determinar la eficacia del bloqueo anestésico transforaminal en pacientes con dolor radicular secundario a hernia de disco lumbar, a través de un estudio prospectivo observacional. Métodos: El estudio fue compuesto por 176 pacientes de una clínica privada sometidos al bloqueo anestésico transforaminal realizado por un único cirujano de la columna. Los pacientes fueron re-evaluados al cabo de dos semanas, tres meses y seis meses con respecto a la mejora del dolor que se irradia a las extremidades inferiores. En caso de persistencia de los síntomas los pacientes podrían optar por realizar un nuevo bloqueo anestésico y mantenimiento de fisioterapia o someterse a una microdiscectomía convencional. Resultados: Al final del seguimiento de seis meses de los 176 pacientes, 116 tuvieron resultado favorable (95 luego de sólo un bloqueo y 21 después de dos bloqueos) y en solamente 43 pacientes la cirugía fue necesaria. Conclusión: Los resultados de nuestro estudio sugieren un efecto positivo del bloqueo anestésico transforaminal para el tratamiento de la ciática en los pacientes con hernia de disco lumbar.

OBJECTIVE: To determine the application of the modified Oxford score among patients with proximal femoral epiphyseal slippage (PFES) as an aid to indicating prophylactic surgical treatment on the contralateral hip. METHODS: Retrospective analysis on the medical files of patients attended at the institution where the authors work. From these, patients attended between 2008 and 2011 who presented unilateral PFES and were followed up for a minimum of two years were selected. Patients were excluded if they presented endocrine disease, metabolic disease, Down syndrome or radiographs that were inadequate for determining the modified Oxford score. The initial radiographs received scores ranging from 16 to 26. Statistical analysis was used to determine whether the scoring was predictive of future development of contralateral slippage. RESULTS: Among the 15 patients with unilateral PFES that were selected, five (33.3%) evolved with contralateral slippage. The patients were divided into two groups. Four patients were considered to present risk and three of them developed contralateral slippage. In the group that was considered not to present risk, there were 11 patients and two of these evolved with contralateral slippage. Thus, there was a tendency for the patients in the group that developed the disease to differ from the group that did not develop it, in relation to the risk classification. CONCLUSION: Although application of the modified Oxford score was not statistically significant in our sample, we noted a tendency toward contralateral slippage among hips with low scores.

MÉTODOS: Análise retrospectiva dos prontuários dos pacientes atendidos na instituição na qual os autores trabalham. Foram selecionados aqueles com um tempo de seguimento mínimo de dois anos, atendidos de 2008 até 2011, que apresentaram EEPF unilateral. Os critérios de exclusão foram pacientes com doença endócrina ou metabólica, síndrome de Down e aqueles com radiografias inadequadas para determinar a pontuação no escore de Oxford modificado. As radiografias iniciais receberam uma pontuação que varia de 16 a 26. A análise estatística foi usada para determinar se a pontuação foi preditiva do desenvolvimento futuro de deslizamento contralateral. RESULTADOS: Dos 15 pacientes selecionados com EEFP unilateral, cinco (33,3%) evoluíram para o escorregamento contralateral. Os pacientes foram divididos em dois grupos, quatro pacientes foram considerados de risco e desses três desenvolveram o escorregamento contralateral. No grupo sem risco havia 11 pacientes, dois evoluíram para o escorregamento contralateral. Nota-se assim uma tendência de que pacientes do grupo que desenvolveu a doença difiram do grupo que não desenvolveu em relação à classificação de risco. CONCLUSÃO: Apesar de na nossa amostra a aplicação do escore de Oxford modificado não ter sido estatisticamente significativa, notamos uma tendência para o escorregamento contralateral nos quadris com escore baixo.

INTRODUCTION: Number 3 cleft or oral-nasal-ocular cleft is a well-known entity that was described by Morian over a century ago. This malformation is a paranasal-medial orbitomaxillary cleft running across the lacrimal segment of the lower eyelid and over the lacrimal groove. The Tessier number 3 naso-ocular cleft represents one of the most difficult and challenging malformations to correct for the reconstructive surgeon. We have conducted a retrospective analysis of our series consisting of 21 cases. OBJECTIVE: The objective was to review the functional outcome and aesthetic results of the different techniques applied for each case. MATERIALS AND METHODS: From 1997 to 2007, 21 patients with a Tessier number 3 cleft were treated in our craniofacial units. The clinical findings, tomographic studies, and surgical procedures were reviewed and analyzed. We have discussed our protocol of the treatment. RESULTS: We have treated facial malformation in 2 craniofacial centers. Fourteen patients were evaluated in the first year of their life, with an average age at presentation of 3 years. Twelve patients were female, and 9 were male; 6 patients had amniotic bands in limbs, 5 patients had an association with Tessier number 11 cleft, 3 patients with number 9 cleft, and 1 with number 7 cleft. Related to cleft lip, 10 patients had bilateral cleft lip, and 8 patients had unilateral cleft lip. Three patients did not have any involvement of the upper lip. The alar base was deviated upward in 19 patients, 11 cases had severe anatomic alteration with the lateral border of the ala above the medial canthus, and 8 cases had a mild dislocation. Nine cases of lacrimal duct obstruction and 8 cases of lacrimal duct extrophy were identified. Twelve patients had a lower eyelid coloboma of varying grades, and there were 2 cases of microblepharia. Aiming the soft tissue reconstruction, eyelid, nose, and upper lip were evaluated regarding their position, absence of tissue, and position of medial canthus and ala. Twelve of our patients underwent correction in the same moment, their medial canthus rotated upward and the ala downward, using the contralateral side as the reference. The lip was treated using a Millard-like technique. Neo-conjunctivorhinostomy was performed in the same moment in 2 patients or later in 1 case. Four patients had plagiocephaly due to the cranial involvement, and they were submitted to cranioplasty. Three had neurosurgical approach and advancement of the frontal bandeau. One adult patient received an acrylic plate to reshape the frontal area. CONCLUSIONS: Tessier number 3 cleft is one of the most difficult and challenging malformations to correct for the reconstructive surgeon. Besides the difficulties of its treatment, patients with Tessier number 3 cleft may achieve good results when the team has good skills.
